Many of the forms on the 4 sub-pages were donated to the website by office managers, billers, coders and physicians.  (Since there are hundreds, we broke them up in to 4 pages, alphabetically)

If you are looking for one of the hundreds of forms, appeal letters, fee schedules, etc, you can click on the alphabetical links below.  For instance, if you want an excel file of the ICD-10 codes, showing every one of the ICD-10 codes for 2018, with it's short description - you would click on the G-O page below.   If you are looking for FREE SUPERBILLS - click on the Superbills page below.  Free appeal letters will be in the 1-9, A-F page and HCFA 1500 forms or CMS forms will be in the appropriate pages.
